The course of cardiac surgery was organized in May 2010 at the Centre of Continuing Education in the Department of “Surgery” with courses of “Cardiac surgery, anesthesiology, critical care medicine and ultrasound diagnostics”. The Academic Board of KazNMU named S.D. Asfendiyarov in 2013, the course was reorganized into the Department of Cardiovascular Surgery.

 

Purpose – training of doctors who graduated from a medical institution of higher education on a specialty “Medicine” or “Pediatrics”, for the specialty ‘Cardio-vascular surgery” in the institutions of practical public health.

Yegemberdiev Tolegen Zhanylbekovich – MD, Professor, Head of the cardiac surgery course of KazNMU, surgeon of the highest qualification category of cardiovascular surgery. 38 years of scientific and practical work. For a long period of time, T.Egemberdiev was a responsible performer and a leader of All-Union and republican scientific topics dealing with current problems of cardiovascular surgery. He took an active part in the development and introduction of new types of reconstructive surgery on heart, aorta, arteries and veins.

The many year research papers of T.Yegemberdiev were closely associated with the research work in the field of cardiovascular surgery (receiving artificial heart valves and blood vessels) of the Russian Scientific Center of Surgery named after B.V.Petrovski, and the Medical Academy named after I.M.Sechenov (Moscow), with well-known Russian scientists – Academician B.A.Konstantinov, professors S.L.Dzemeshkevich, Y.Belov, I.A.Sychenikov, V.A.Kozlov, and A.I.Ivanov. The proposed by T.Yegemberdiev bioprosthesis of vessel of new type in 1979 is widely used in the surgical treatment of diseases of the aorta and arteries, as well as kidney allotransplantation, not only in Kazakhstan but also outside the country.

T.Yegemberdiev put a lot of effort and work into the organization of the Republican Center of Vascular Surgery, Vascular Surgery regional centers in Shymkent, Semey, Pavlodar, Aktau, Atyrau and Zhambyl regional center of Vascular Surgery (1978), as well as training angiosurgeons of the listed centers by teaching them directly in the operating room.

T.Yegemberdiev is a bright scholar, selfless mentor puts a lot of work and effort into the training of young scientists and surgeons in the field of cardiovascular surgery. He is a member of the Republican specialized scientific council (D.09.07.01) granting the degrees of a Doctor of Medical Sciences in the field of “Surgery” and “Cardiovascular Surgery” at the Cardiac Center named after A.N.Syzganov, and from 2002 T.Yegemberdiev is an MD, professor of the Kazakh National Medical University named after S.D.Asfendiyarov.

T.Yegemberdiev is the surgeon of the highest category, Excellent of Health protection (1992), a member of the Russian Society of Cardiovascular Surgeons (1992), Angiology and Vascular Surgery (1994), Association of Cardiovascular Surgeons of Central Asia and Kazakhstan (2003), Kazakhstan Association of Phlebology (2006).

Professor T.Yegemberdiev has 5 patents and inventors’ certificates, the author of more than 200 scientific papers published in international and national medical journals.

Seithan Dzhoshibayev is an Academician of the Academy of Preventive Medicine of the Republic of Kazakhstan, corresponding member of the national Academy of Science of Kyrgyz Republic, Honored Doctor of Kyrgyz Republic, Doctor of Medicine, professor.

Seithan Dzhoshibayev was born in June 9, 1939 in Zhambyl region. After finishing Kazakh, secondary school in 1956-62, studied in Medical Institute in Bishkek city (Kyrgyzstan). For 5 years, he has worked as a general surgeon in regional hospitals in Kyrgyzstan.

In 1967 -70 pursued postgraduate studies in cardiovascular surgery in Moscow, thereafter worked as a cardiovascular surgeon in Republican clinical hospital in Bishkek city.

In 1976, he arranged Seithan Dzhoshibayev and administered Republican Cardiac Surgery Centre in Kyrgyzstan, which by the nature of its activity was one of the eight among 24 centers of the Soviet Union.

In 2004, he arranged Research Institute of heart surgery and transplantation, having been administering it last 2 years.

In 2007, Seithan Dzhoshibayev moved to Taraz city (home) and arranged «Centre of Cardiac Surgery» LLP, where he could approve his organizational and professional skills. For the first time within the southern region (Zhambyl region, South-Kazakhstan region, Kyzylorda region) complex cardiac surgeries were carried out. Medical specialists of the field – therapists, cardiologists, pediatricians, intesivists have research/practice conferences devoted to the issues of diagnosis, treatment of cardiac diseases, intraoperative and postoperative care. Up to now, he operates 60-85% of surgeries in Center of Cardiac Surgery by himself.

Seithan Dzhoshibayev prepared 9 Candidates of Medical Science and Doctor of Medicine, 6 of them are from Kazakhstan.

Seithan Dzhoshibayev undertakes measures against common diseases, which are cardiovascular diseases, within the program of opening in 2009 the Research and Clinical Centre of heart surgery and transplantation with 80 beds in cooperation with foreign investors in Taraz city, which will serve as a regional center and represent Kazakhstan to global community.

Seithan Dzhoshibayev is justly rewarded with certificates of honor by Ministry of health of   Kyrgyzstan, Kazakhstan, Mongolia, with the following badges –  “Expert of Healthcare of the USSR”, “Expert of Healthcare of the Republic of Kazakhstan»,  “Honorary citizen of Karaganda city” and Ryskulov district of Zhambyl region, with medals “Dank” of Kyrgyzstan, “Eren enbegi ushin” of Kazakhstan, in 2005 was recognized as “Person of the Year 2003” and “Man of Distinction of the 21 century”.

Seithan Dzhoshibayev is a responsive person, attentive and readily available to others, a family man, a man of integrity and principle, consistent in his actions and decisions.

Orazbek Sakhov is a Candidate of Medical Sciences, associate professor, one of the leading specialists in the field and a member of Expert Advisory body for Cardiovascular Surgery in Eastern and Central Europe. Head of Roentgenendovascular Surgery Department at Heart Centre, Almaty city.

Kuatbekov Kairat Niyetalievich – is a Candidate of Medical Sciences, associate professor, surgeon of the highest qualification category of cardiovascular surgery; Head of Department of Children’s cardiac surgery “Centre of perinatology and Children’s cardiac surgery”, Almaty city.

In 2005 completed a two-month specialization course “Surgical management  of congenital heart defects” based on Children’s hospital Górnośląskie Centrum Zdrowia Dziecka i Matki im. Jana Pawła II (Katowice, Poland); In 2005, he was awarded with  a badge “Kazakhstan Respublikasy  densaulyk saktau isiniz yzdigi”; From 2007 to 2010, he was a student of full time doctoral program for cardiac surgery. In 2011, he completed advanced training on the theme “Children cardiac surgery and neonatal surgery” based on Novosibirsk Research Institute of blood circulation disorder named after Ye. N. Meshalkin; From 14.11.2011 and till present has been working as a Head of Department of Children’s cardiac surgery “Centre of perinatology and Children’s cardiac surgery”, Almaty city. He has authored over 106 publications, 7 inventions and 2 guidelines.

Almat T. Koldasbayev is a Candidate of Medical Sciences, associate professor. Director of Heart Centre, Almaty city. He is one of the leading heart surgeons, completed full course in famous international centers for cardiac transplantation. He has authored over 50 publications in the field of acquired heart valvular diseases

Nursultan A. Kospanov is a Candidate of Medical Sciences, teaching assistant. Head of Department of angiosurgeon at National Research Centre named after А. N. Syzganov, Almaty city. He is a specialist in a reconstructive surgery of aorta and main arteries. He has authored over 50 published works

At this time (2013-2014) is training in the Clinic cardiac, thoracic and vascular surgery of Lithuanian University of Health Sciences (Lithuania, Kaunas) in Cardiac Surgery – Surgery of the root, ascending and aortic arch, reconstructive plastic operations on heart valve  and heart transplantation ( by program “Bolashak”).

Courseware

In 2013-2014 academic years, students and residents study the following disciplines:

  1. The students are taught: “Cardiac surgery”, “Vascular surgery”, “Vascular surgery with Roentgenendovascular Surgery”, “Retraining courses”, “Perfusiology”. Advanced courses “Urgent cardiac surgery”, “Minimally invasive angiosurgeon”.
  2. Residents are taught appropriate disciplines by specialty “Cardiac surgery, including Children cardiac surgery”, “Vascular surgery, including Children vascular surgery”.
  3. Residents are taught complementary sciences by specialty “Cardiac surgery, including Children cardiac surgery”, “Cardiac surgery”, “Interventional cardiology”.

Full-time education, including lectures and seminars according to subject-schedule, final exams.

Teaching materials for specified disciplines are provided in the form of Sample programs, Work academic programs, Syllabuses, notes and presentations, prepared according to Informative No.8, No.9.

The following materials in Russian language for seminar lessons are prepared at the department and printed typographically:

At the Department, during practical classes the following interactive teaching methods are used:

  1. “Visualization of modern surgery technologies” directly in operation room, in cardiac surgery, aorta and large vessels using machines of artificial blood circulation (CPB pump) and without CPB pump, as well as video films, CD by main sections.
  2. “Visualization of modern roentgenendovascular interventions” in aorta, coronary and main arteries (stentings, destructions of nervous tissues of renal artery, hybrid technologies at complex heart and vessels disease) directly in operation room, on video films.

Introduction of new technologies in educational processprepared for registration of regulations of implementation.

                                              Clinical service

Under assistance of the head of “Cardiosurgery” course Yegemberdiyev T.Zh. together with docents and assistants of the course were made 3827 operations, were consulted 4315 patients.

At the City cardiologic center of Almaty city were made coronography – 3146. 36% of patients (1131 persons) were referred from intensive care unit and aggressive treatment unit. The age of patients was from 19 to 92 years, average 60.2±11.4 years. Men are majority – 2159 (68.6%).Main diagnoses under the reference to coronography were: myocardial infarction (41.1%) and acute coronary syndrome (36.9%).   Evolving stenocardia of 21.9%, congenital heart disease – 0.1%. 2900 of coronographies (92.2% of total amount) were made by transradial way that allowed to strongly decrease the quantity of complications at the site of operation.

 

By the results of coronography the conservative treatment was recommended to 39.2% of patients, сoronary artery bypass surgery – 20.7% and percutaneous coronary intervention – 40.1%. Thus the correlation of these methods is 2:1:2, in comparison with the last year the quantity of percutaneous coronary interventions is increased. This correlation in 2011 was 5:2:3.

Totally for 2010 were made 1260 percutaneous coronary interventions with implantation of 1685 stents (in 2011 – 1088 percutaneous coronary interventions with implantation of 1605 stents).

A balloon angioplasty without implantation of stents was made to 53 patients (4.2%). To 1118 patients (88.7%) were implanted 1-2 stents. To 89 patients (7.1%) were implanted 3 and 4 stents, these patients were distinguished by extension of lesion of coronary bed and necessity of full recanalization of vessels’ lumens. In average per one patient were implanted 1.3 stents, in 2011 this indicator was 1.5

Lethal outcomes after stenting were indicated at 30 patients (2.4% of 1260 percutaneous coronary intervention). In comparison with 2011 a slight decreasing of lethality is marked (in 2011 – 2.6%, i.e. 29 of 1088 percutaneous coronary intervention). All cases with lethal outcome were discussed with cardiologists, cardiosurgeons, resuscitators and departments’ employees.

At the “Center of perinatology and pediatric cardiosurgery” of Almaty city within the department “pediatric cardiosurgery” from 15.01 through 26.12.2012 totally operated 207 children (from 3-days old to 15-years old) with different forms of congenital heart defects, including 94 (45.4%) children – aged up to one year (including 12 (5.8%) – newborns up to 28 days old), 53 (25,6%) children – from 1 to 3-years old and 60 (29%) children – over 3 years old. 174 operations (84,1%) were made under the condition of artificial blood circulation with pharmaco-hypothermic cardioplegia. Hospital lethality was 6.7% (14 events). Level of complexity of pediatric cardiosurgery operations based on basic scale of Aristotle is 1-4, quanity of points 1.5-15.0.

 

 

Minimal weight of patient at “closed” operations was 1360g., at height 40sm., and at operation under the conditions of artificial blood circulation with pharmaco-hypothermic cardioplegia – 2047g., height – 47sm.

Among operated newborns there were vents of practical interest. Execution of these operations were not impossible before due to some circumstances:

– These newborns and immature children in the age of 14, 17, 19, 21, 27 days with critical Congenital Heart Defects. Transposition of great vessels (3); Total anomalous pulmonary veins drainage (2); Hypoplasia of arch/or Aorta angusta with  ventricular septal defect (2);  Single ventricle with tricuspid atresia, and agenesis of the pulmonary valve with a restrictive atrial septal defect (2); truncus arteriosus (1); Patent ductus arteriosus. Prematurity of 32weeks (1); Aorta angusta + Patent ductus arteriosus;

– palliative and hemodynamic Corrective surgery in children with difficult-combined Congenital Heart Defects (common ventricle, obtuse marginal artery, ALA, junctional cyst unbalanced form with TMS, etc.) were performed in 15 children;

– initial radical correction of blue Congenital heart defect. Fallot’s tetralogy was sucessfuly made in 11 children, including 8 – aged up to one year;

– radical correction of junctional communication was made in 6 children, three of them aged up to one year;

– successful radical correction of rare Congenital heart defect. Subvalvular membranous stenosis of the aortic valve in combination with a ventricular septal defect (2 children) etc.;

– rhabdomyomas successful removal of the bulk of the right atrium and right ventricle with tricuspid valve plasty suture in artificial blood circulation in children aged 46 days of life ;

– held a successful sesqui-ventricle correction (transannular valvuloplasty LA and o.Glenn bi – cavopulmonary anastomosis) of complex CHD. The anomaly of the tricuspid valve (stenosis) in combination with a critical stenosis (1 -2mm), pulmonary valve and a PDA in child of 2 months old;

– made a few simultaneous operations (one-stage correction of the CHD and other surgical pathology (diaphragmatic hernia, phimosis, etc.);

– 2 children operated on for CHD were born induced by IVF ;

– associated serious medical problems like Down syndrome – were found in 16 (7.7 %) children;

– Plastic VSD with synthetic patch to the abandonment of the fenestration was performed in children with CHD. VSD with high LH 3B – 4g. (at 3);

– completed successful implantation pacemaker (dual chamber), “Medtronic” with bipolar epicardial electrodes in a child aged 6 months with postoperative AV block 3st .

– about coarctation of the aorta and aortic arch hypoplasia in conjunction with PDA successfully operated on 3 children (including 1 – newborn to age 14 days of life) etc.

 

At “Cardiac Center “, Taraz city, performed surgeries: removal of coarctation of the aorta (5), pulmonary valvuloplasty (13), the closure of atrial septal defects (70). Interventricular (62) walls of the heart, the correction of tetralogy of Fallot (58), atrioventricular communication (12) – to children from 1 year old, mitral (159), aortic (24) and tricuspid (2) dual chamber prosthetic valves and adjusted the tricuspid valve heart ( 49). Plastic operations on mitral (24) and aortic (21) valve. Closed mitral commissurotomy (8). Coronary artery bypass surgery (59) at coronary heart disease and other operations in comorbidity of heart.

International cooperation is implemented under agreement for international cooperation with the following organizations:

– Lithuanian University of Sciences and Health (Lithania, Kaunas)

Hospital of cardiac, thoracal and vascular surgery. Supervisor is Rimantas Benetis, professor. There is an agreement for educational and scientific information international cooperation. Candidate of Medical Sciences, an assistant of the department Yegemberdiyev Zh. T. undertakes an internship for cardiac surgery for 2013-2014.

From 2012, based on the cooperation agreement with “BURC MEDICAL” LLP (Turkey) we are implementing a cooperative educational program and arrangements for scientific and information exchange in cardiovascular surgery.
